Kaiser Permanente's network doctors and claims administration offers fully insured plan options through CEBT. With several HMO and DHMO plans each employer group within a designated service area can choose to offer these alongside any other CEBT plan options or choose to offer Kaiser plans on their own.
Covered Drug Lists
For a list of covered drugs, please access the Drug Formulary (Covered Drugs) on the kp.org website. Once you are directed to the page, click the Large Employer Group (50+ Employees) to access the drug lists covered under your plan.
CEBT Benefit by Trust is fully committed to providing accessible facilities, elements and
